1.  Proof of Age (Copy of Birth Certificate, Passport, or Real ID). If your athlete played in Fall of 2024 we will not require you to provide a Birth Certificate.

2.  2024/2025 Full Report Card

3.  2x2 recent picture of athlete (no hoodies, caps, masks)

5.  2025 Participant Contract and Parental Consent Form 2025 participant contract and parental consent form.pdf

6.  $350 AND $375 Check or Completed Credit Card Auth. Form as an Equipment Retainer and Volunteer Time Retainer. This ensures we receive our equipment and each family full-fills the volunteer requirements of 10hrs per family. deposit_cc_authorization.pdf
